Please bring all required documents and be prepared for a comprehensive consultation.

Please bring:

  • Referral from your doctor
  • Copies of recent test results and hospital discharge summaries
  • Relevant x-rays, CT/MRI/PET scans, angiograms
  • List of current medications
  • Medicare/DVA/Pension/Private Health Fund cards

Please note: Sometimes, due to the nature of our work, there are often emergencies resulting in appointment postponement. If this occurs you will be contacted as soon as possible and rescheduled within a week.
